Self-report health questionnaires data is collected to obtain behavioral and psychological information as well as medical history and family history of diseases. It includes medical history, smoking status, alcohol consumption, diet, physical activity, socio-demographic characteristics, sleep quality, depression, anxiety, and quality of life. This will help us to understand, analyze, and monitor patient’s health more comprehensively. We try to make sure to use all internationally standardized questions, and the Web-CASI (Computer Assisted Self Interviewing), our new health questionnaire system, will guarantee precision and accuracy of the data.
